2023-24 Sports Medicine Staff


Welcome Student-Athletes! 

Prior to participation in NCAA athletics or ACHA Ice Hockey at Calvin University, there are several requirements you must meet before you may begin participation in team activities like tryouts, practices, or strength/conditioning. All medical requirements, including any necessary paperwork and instructions, can be found on this website. Your coach will contact you with information about any non-medical requirements. 

Due dates 

Please be sure to read all instructions carefully as incomplete information or missed deadlines may cause delays to your athletic participation. Due to anticipated start dates for all sports, we ask that you complete your information and submit all documentation for review by August 1 to ensure that there aren’t any delays to starting your athletic participation. 

Documentation and physicals 

As a student-athlete you fall into one of two categories, each with slightly different requirements: 

Incoming/Transfer Student-Athlete?» 
If you have never played an NCAA sport or ACHA hockey at Calvin University, you should complete the requirements for Incoming/Transfer Student-Athletes?» 

Returning Student-Athlete?» 
If you have previously participated in an NCAA sport or ACHA Ice Hockey at Calvin University, you should complete the requirements for Returning Student-Athletes?» 

2025-26 physical information, including updated links and instructions, will be available in June 2025. Email communication will be delivered to all incoming and returning student-athletes from Sports Medicine at that time. Please reach out to Jessica Rhodes at jr243@calvin.edu with any questions.

Hours and location 

All sports medicine services are located in the Van Noord Arena Sports Medicine Office (VN 137) on the ground floor (just off the northwest corner of the main court). This facility provides treatment and rehabilitation to student-athletes. 

Monday - Friday 
10 a.m.–2 p.m.: Walk in evaluations 
2 p.m.–6:30 p.m.: Practice/game coverage (no new evaluations) 

Saturday 
By appointment only or as games and practice require 

Sunday 
Closed 

Welcome Visitors! 

The Van Noord Arena Sports Medicine Office (VN 137) will be open at least 2 hours prior to your game and as needed following the game. We will supply your team with water, cups, and ice on the bench for games. Please let us know if you need any additional supplies. 

If your team will be traveling without a Certified Athletic Trainer, if you will be using our facilities outside of normal game play, or if your team will need anything beyond the previously mentioned supplies, please contact us and we will do our best to accommodate the needs of your athletes. 

In the case of emergency, injury, or illness while your team is in Grand Rapids, we recommend these local hospitals and medical centers » 

Our Mission 

It is the mission of the sports medicine staff to provide high-quality holistic health care to our student-athletes, to provide a professional health care environment, to serve as advocates for the health and well-being of our student athletes, and to be proactive in managing their healthcare and general well-being. 

Recognizing that a holistic approach involves mind, body, and spirit, we work closely with several other offices at Calvin University to ensure that you receive the care you need to be successful as a student-athlete. Our office uses an integrated team approach of certified athletic trainers, physicians trained in sports medicine, physical therapists, and other qualified health care professionals to provide injury prevention, assessment, treatment, and rehabilitation to Calvin University student-athletes participating in NCAA and ACHA intercollegiate athletics.
