Incoming/Transfer Student-Athlete Checklist
Information on your initial sports physical and concussion testing will be sent to you by the Calvin Sports Medicine Office. This will include the date and time we have scheduled you to complete your physical with a team physician. Please note: There will be a one-time, $25 fee charged to your student account to offset the administrative costs associated with sports physicals. Additional Documentation
You may need to submit additional documentation for certain medical conditions. If you had a serious injury requiring surgery in the past year or have an ongoing medical condition (i.e., asthma, diabetes, epilepsy, etc.), please submit a letter from your surgeon or managing physician that clears you for full participation or outlines any restrictions you may have.
Substances that are generally purported to be performance enhancing and/or potentially harmful to the health and safety of the student-athlete have been banned by the NCAA. This means that medications commonly prescribed by physicians for the treatment of ADHD and asthma, among others, are banned without a medical exception.
